COBRA Letters -
* All COBRA letters are mailed first class mailing to the last known address. Employers should obtain the current address for employees and covered dependents when they incur a COBRA Qualifying Event.
 

Types of COBRA letters

COBRA Rights Letter Balance Due Letter
Qualifying Event Letter Termination Letters
Invoice Letter & Coupons Conversion Letters
 
COBRA Rights Letter is issued when an employee, or spouse of an employee, first become covered under the employer's health plan. The COBRA Rights letter is an informational letter that explains the beneficiary's rights and responsibilities under the COBRA law. Separate letters are issued if there are separate addresses.
 
Qualifying Event Letter is issued when health insurance coverage is lost due to a COBRA Qualifying Event. The letter lists each qualified beneficiary who is eligible to continue health benefits, the date their benefits ended, the time frame to elect, and the cost per benefit plan option available. The letter explains COBRA eligibility and the length of potential COBRA coverage. It also provides an election form and gives a customer service phone number for their COBRA questions. Separate letters are issued if there are separate addresses.
 
Invoice Letter & Coupons. An invoice letter and coupons are sent to COBRA continuants when their COBRA application has been accepted and whenever their monthly premium amounts change. The invoice letter explains the initial and monthly payment due dates and grace periods, and reiterates COBRA eligibility.
 
Balance Due Letter is sent to COBRA continuants whenever a payment is received that is short by an "insignificant amount". The letter informs them of the short payment and allows an additional 30 days to remit the difference, as required by law.
 
Termination Letters are sent to COBRA continuants whenever COBRA coverage terminates. It lists the date their COBRA coverage terminated and the reason.
 
Conversion Letters are issued to active COBRA continuants within 180 days of their projected termination date notifying them of their right to convert to an individual policy. COBRA does not require plans to have conversion rights. Since not all plans have conversion rights, conversion letters are only sent when applicable.
